College Admission Requirements

Bring the following documents upon admission.

3 copies Certificate of Live Birth

3 copies 2x2 ID Picture (Recently taken close up shot)

For Freshmen:

  3 copies SHS Report Card (Form 138-A)

  3 copies Certificate of Good Moral

For Transferee:

  3 copies Honorable Dismissal

  3 copies Transcript of Record

For Prof. Ed. Adding Units:

  3 copies Transcript of Record

Enclose the documents inside a Clean Long White Folder


Enrolment Steps

1. Submit Documents to Admission Office. Register for College Entrance Examination.

2. Take & Pass the CEE. Claim result slip.

3. Picture Taking in Admission Office.

4. Present result slip to the department of your course of choice.

5. Pass the interview and submit any additional documents required by the department.

6. Secure Department Enrolment Form.

7. Bring Enrolment Form to Cashier Office, Billing Office, and Registrar's Office for Recordkeeping and Validation.

8. You are officially enrolled. Await class schedule from your respective department.
